Individual Camper Registration Forms

ALL FORMS MUST BE PRINTED OUT AND EITHER MAILED IN OR BROUGHT IN PERSON TO THE IN PERSON REGISTRATION

Please come prepared with your forms already printed as we will have a limited amount available

The following links will allow you to print the forms needed for registration at camp.  Please remember you need 1 of each form for each camper, tot lot or boys unit that you will have attending camp.

Adult Volunteer Forms

You know you want to help and we can't wait to have you on Staff!  The following links will allow you to print the needed forms for you to be registered as an Adult Volunteer at the 2008 Simi Valley Day Camp.  

Do not mail into Santa Barbara Address.  Mail to the Simi Valley Address or bring it in person.  Please include your shirt size on top of form.
