Registration Page:  A camp name is a special nickname your camper would like to be called.  If they have attended camp before, your camper might already have a special name and may re-use it.  All staff including your directors ,better known as Sarge (Tanjia) and     will  have special camp names.  These will be the names that both staff and campers will call each other.

Please list no more than 1 friend that your camper would like to be in a unit with.

To Whom a Girl May Be Released: Please do not forget to put any and all parents names on this form.  Also include any adults who might pick your child up.  This includes any carpools you might arrange with parents of other campers. DO NOT FORGET YOURSELF!

Emergency Medical Release:  Please do not forget to put your insurance information down as well as an emergency contact even if this person is yourself.

Consent to Provide Medication:  There are two parts to this form.  1)PERSONAL MEDICATION NOT DESCRIBED BELOW;  This is for any prescription medication or other over the counter medication you wish your child to recieve while at camp.  If your child has none you may leave it blank. 2) PERMISSION TO ADMINISTER CAMP-SUPPLIED MEDICATIONS;   Please initial any and all items you will allow us to administer to your child while at camp.  This allows us to deal with those minor hurts without contacting parents to obtain permission each time.

With all forms please remember to fill them out in their entirety.  An incomplete form will cause your application to be held up until all information is completed on the form.  All forms must be signed to be considered complete.  If you should have any further questions please do not hesitate to contact one of the camp  directors.
